The Problem With Standard PT

Most treatment protocols address the symptom.

Conventional physical therapy is largely diagnosis-driven: a shoulder diagnosis receives a shoulder protocol, a lumbar diagnosis receives a lumbar protocol. This approach works for straightforward, isolated injuries. It consistently fails for the patients who need the most help — those with complex, chronic, or recurring pain that doesn't resolve on a standard timeline.

The reason is simple. In the human body, the source of pain is rarely where the symptoms are. Tension in the posterior cervical fascia can drive shoulder pain. A neural sensitization pattern that began at the greater occipital nerve can produce symptoms from the occiput to the fingertips. A compensatory movement strategy adopted years ago to protect one structure is now overloading three others. Standard protocols aren't designed to find any of this.

01 Myofascial System

Fascial restrictions and trigger points are among the most common — and most consistently overlooked — drivers of chronic pain. The Silan System includes proprietary myofascial frameworks, including the Splenius-Serratus-GON sequence for cervicogenic and upper extremity presentations.

02 Musculoskeletal System

Muscle isolation to eliminate compensation is a core clinical principle for strengthening. The Silan System systematically identifies and retrains dysfunctional movement patterns — because the body's compensatory strategies, while protective in the short term, can become a source of pain over time or the reason it persists.

03 Autonomic & Nervous System

Pain is not purely a mechanical event. The Silan System incorporates nervous system regulation as an active treatment goal when necessary, such as in chronic pain conditions— using novel techniques to move the patient's nervous system from threat-response to healing-response.

guidING PRECEPTS FOR every treatment session:

  1. No two bodies are the same. Every treatment plan is built from assessment findings — not from the diagnosis on the referral. What works for 80% of patients with a given presentation will fail the 20% whose underlying drivers are different. The Silan System is designed to find and treat the 20%.

  2. Gentle-first is a clinical standard, not a style preference. The nervous system responds to threat with guarding, splinting, and sensitization. Aggressive early intervention can compound this. Gentle, skilled manual therapy — applied with precision — produces faster and more durable results than force.

  3. The remote source principle. Assessment always follows the symptom upstream. A shoulder that doesn't respond to shoulder treatment has a source elsewhere. The Silan System's assessment protocols are built to find it.

  4. Test, treat, re-test. Every intervention is evaluated in real time. If the response is not what was expected, the hypothesis changes and the approach changes. This is clinical reasoning, not protocol execution.

  5. The Rule of the Rep. Quality of movement always supersedes quantity. A single correctly isolated, fully conscious repetition produces more neuromuscular change than twenty compensated ones. Re-education, not repetition, is the goal.

  6. Active aging should be the norm. Decline is not inevitable. The body can build muscle until the end of life. Pain is not a tax on aging. It is information — and it can almost always be addressed.
